OPERATIONAL GUIDELINES FOR THE FACULTY REVIEW BOARD

The fundamental responsibility of the Faculty Review Board is to perform an Ombudsman function for members of the Faculty who have been unable to resolve specific problems through the normal administrative channels. Formal guidelines are described below.
  1. Membership, terms of service, meetings
    1. The Board consists of five elected members of the Faculty, excluding the Director and Assistant Directors, serving two year, staggered terms. Elections are held each year by mail ballot, either printed or electronic.
    2. The Chair will be the continuing member of the Board with the longest Libraries' service. In the event the Chair cannot complete his/her term, or if a conflict of interest arises, the continuing member with the next longest service will serve as Chair.
    3. Meetings will be called as needed by the Chair.

  2. General Principles: the Faculty Review Board shall operate under the following general principles:

    1. Any member or members of the Faculty may petition the Board for review of a case.
    2. Each request shall receive prompt attention.
    3. A Board member shall not participate in review of cases where conflict of interest arises.
    4. The Board shall take precautions to insure that the rights and privacy of those involved in any case are protected.
    5. The Board shall not consider a request for review while an administrative review of a specific problem is in process.

  3. Prerequisites to review by the Faculty Review Board

    The completion of a full review through appropriate administrative channels is a prerequisite to formal consultation with the full Faculty Review Board. The concerned Faculty member(s) have the responsibility to initiate the administrative review and to provide some evidence that a review has occurred.

    During the administrative review the Faculty member(s) may discuss the case informally with individuals on the Board or the Libraries' human resources staff.

  4. Review Process

    1. Faculty members may submit a request for review to any member of the Review Board. The request should be submitted within a calendar quarter following the final administrative decision regarding the problem.

      The review Board may waive the time limit at the request of the initiating Faculty member(s) if circumstances warrant.

      The written request for review should include a statement of the case encompassing:

      1. the details of the case, including specific examples when appropriate;
      2. a description of the nature of the possible resolution sought by the Faculty member(s);
      3. a statement of the decision arrived at by the final administrative review, as understood by the Faculty member(s).

    2. The Review Board will request from all those involved in the case additional information that is considered necessary to conduct the review.

    3. The Review Board may refuse to review a case that it considers outside its scope. In this circumstance, the Review Board shall provide a response in writing which will include the reasons for the refusal to review the problem and will refer the Faculty member(s) to the appropriate review or appeals procedures.

    4. A Faculty member may withdraw a request for review at any time. In cases involving more than one Faculty member, any individual may withdraw from the review. However, the review will continue unless all concerned Faculty request the review be dropped.

    5. The formal review shall include:
      1. Consultation with those involved in the case, including relevant third parties.
      2. Written documentation when necessary.
      3. The final report of the Board encompassing the majority decision and any dissenting opinions.

    6. The final report including recommendations shall be given to the Director of Libraries and the initiating Faculty member. Administrators involved in the original administrative review shall be notified in writing that a report has been made and submitted to the Director.

    7. The Director of Libraries shall meet with the Review Board to discuss its report and to discuss planned administrative action concerning the problem.

    8. The Review Board will meet with the Faculty member(s) who requested the review, within a calendar quarter of submission of its final report, to assess progress toward resolution of the case.

  5. Faculty members should be aware that the Review Board's role is purely advisory and is used to provide a disinterested review of specific problems.
