New Hire Paperwork for Students
Congratulations on your job offer from University Libraries. Please complete the following paperwork for your new student position. When finished, you will have completed a total of 7 or 8 documents. Make sure each document is completed, signed where applicable and returned in person to Libraries Human Resources, Room 305 Thompson Library, 1858 Neil Avenue Mall. Make sure to bring with you the appropriate identification documents noted in I-9 form. Also, make sure to bring your referral documentation provided to you by your new supervisor. 1 Hire
Some general demographic information is required for the hiring process. Access the following link, complete the Student Application, and print. Student Application
2 I-9
The federal government requires that your eligibility for employment is verified. Access the following link, complete Section 1 of the form on page four, print and sign. http://hr.osu.edu/forms/emp/i-9.pdf Make sure to bring your “Acceptable Documents”, identification documents, on or before your first day of employment.
3 W-4
Our Tax Office needs to know how your pay should be taxed. We cannot advise you on tax issues. Make sure to follow form directions, use the worksheet and if some claims you on their taxes, you may need consider this in completing the form. 4 Ohio Ethics
As a University employee the Ohio Ethics Code will apply to you. The code is available at this site: http://legal.osu.edu/ethics.php. Access the following link, complete the acknowledgement form and print. The employing unit is "OSU Libraries". http://hr.osu.edu/forms/ethics.pdf
5 Retirement
If you are a student employee enrolled for 6 or more credit hours you may either request an exemption from or contribute to OPERS(Ohio Public Employees Retirement System).
If you are requesting an exemption, complete the Retirement Election form available at http://hr.osu.edu/forms/ben/studentretelect.pdf AND the OPERS Request for Optional Exemption as a Student available at http://hr.osu.edu/forms/ben/exemptstudent.pdf By electing this exemption you are making an irrevocable decision to not be a member of OPERS during this employment period.

If you are a student employee enrolled for less than 6 credit hours you must contribute to OPERS. For example during the summer you may not be enrolled but still work, you must contribute to OPERS during this time. For more information: http://hr.osu.edu/hrpubs/ben/retirestudent.pdf
6 Social Security
Any Social Security Benefits that you may be eligible for may be reduced by retirement benefits you gain from your student appointment. Access the following link, complete the form, print, and sign. http://hr.osu.edu/Forms/Ben/retiressform.pdf
7 Referral Form(s)
Make sure to bring the Student Assistant Employment Referral Form provided to you by your new supervisor. If you are a Federal Work Study student, make sure to print and bring your Student Financial Aid Job Referral Form, accessed at https://sfa-jobs.osu.edu/osu-se/student/home.aspx
Submit seven or eight completed and signed documents in person to Libraries Human Resources, Room 305 Thompson Library, 1585 Neil Avenue Mall. Make sure to bring with you the appropriate identification documents noted in the I-9 form.
